This is another invisible conformer product. This one is a little different in that it has many, many FBMs and PBMs for female characters and breasts.

How it works is, you conform it to Genesis, then you conform shirts and tops to the fixer. Then you render with the Fixer invisible. It eliminates inappropriate clinging and creasing at the center and bottom of female breasts. You don't really need it for bras or the geometries that aggressively pinch the breasts already, and a lot of of my own products already have the appropriate morphs included, but that still leaves many otherwise quality inexpensive sets from DAZ and elsewhere that will never look right without this. Hopefully you can see from the pictures why this is different from the clothing cleavage modifier (which is a morph for Genesis itself, not a conformer).

Is there anyway to make a V4 top/shirt that needs to be auto-fitted to Genesis work with the Sickle Breast Fixer?

Don't use autofit. Convert it with Transfer Utility.

To use TU, load the item and say cancel when it offers to autofit. Then start the Utility by clicking its icon, which looks like an arrow pointing up and to the right.

Choose Genesis, then Clone and Victoria 4 under the left drop-down. Choose your item under the right dropdown. Make sure "reverse source shape" is checked below and add a smoothing modifier. Then click Accept.

If you use TU you must save back to library before doing anything else and reload the item. Then it should work with Breast Fixer.

I had done all that except the "save to library." Can you tell me what that is? All I did was just save the top by itself in a scene fie and then merged it back into my scene.

That won't do it. The second it's converted you need to go file--save as--support asset--figure/prop asset. Then put in the author name and product name and choose Accept. Then delete it from your scene. It will be saved in the place you chose in your library, and after you reload it you can use it with the Fixer.
